| Medicare Plus As from the 1st of July, 2004 Medicare commenced providing a rebate for services provided by Podiatrists treating complex conditions under an Enhanced Primary Care Plan (EPCP) coordinated by a GP. These are conditions that have been present or are likely to be present for at least six months. Complex conditions can include diabetes, heart disease, asthma, cancer and kidney disease or one or more of a whole range of illnesses. Ask your Podiatrist or a Podiatry Assistant for a copy of the Enhanced Primary Care (EPC) Program Referral Form which can be presented to your GP for completion and assessment for eligibility then brought to your next consultation. Patients wishing to receive the Medicare rebate are required to make full payment at the time of their consultation. They will then be provided with a receipt containing all the relevant referral details this must present to a Medicare office to obtain their rebate. NOTE: Your Doctor has the ultimate capacity to determine your eligibility for meeting the criteria for Medicare assistance and the number of visits (maximum of 5) to which s/he thinks are necessary.
